Services and Resources
The goal of home care is to provide assistance and support so patients can live at home with dignity and a sense of independence. Our services and staff are coordinated to meet the needs of homebound patients.
Home care may include:
- Skilled Nursing — Registered nurses provide assessment, care coordination and treatments of a wide variety of illnesses and conditions.
- Physical Therapy — Therapists provide rehabilitation services and therapeutic exercises designed to maximize recovery from illness or injury.
- Occupational Therapy — Therapists provide vocational performance, independent living, sensor motor, cognition, visual and psychosocial support.
- Speech Pathology — Therapists provide swallowing efficiency, cognition, dysphasia, language skills and help with voice disorders.
- Medical Social Services — Medical social workers assess for social, financial, and emotional needs related to illness/care and provide community resource assistance.
- Infusion therapy — Registered nurses administer intravenous infusions for antibiotic therapy, hydration, nutrition and pain management.
- Personal Care — Home health aides provide assistance with activities of daily living, personal hygiene, and other services to promote health and safety.
- Home Telemonitoring — Monitoring of key healthcare data from the patient's home, which is transmitted to their Home Care provider.
- Extended Care — An hourly private pay program offering services when a patient does not qualify under Medicare guidelines or insurance coverage. Services include:
- Respite/Companion Services
- Bath Services
- Homemaker Services
- Blood Draws, Medication Management, Case Management
- Therapy Maintenance Program
- Personal Emergency Response System
