Anticoagulation Management Service

"Blood Thinners"

Some patients are prescribed anticoagulation medication, better known as blood thinners. Our management service cares for patients who are at risk for blood clots or stroke and who are taking drugs such as Coumadin® (warfarin) to slow blood clotting.

Our goals include coordinating therapy with changes in patients' medication, diet and health condition and reducing complications and hospitalizations.

We test blood using a simple finger-stick machine with results in seconds. The pharmacist will then adjust the dose as needed.

A progress note is sent to the primary care physician and our medical director after each visit for review. We are available to manage any patient on warfarin, enoxaparin and fondaparinux for conditions such as atrial fibrillation, stroke, myocardial infarction (MI), cardiomyopathy, deep vein thrombosis (DVT), pulmonary embolism (PE), valve replacement and other conditions that require anticoagulants.
