Case Management

Case Management at Cheyenne Regional is a partnership model including patients, families, nurses, social workers, physicians and other practitioners, caregivers and the community. The Case Management Department coordinates services, personalized hospital care, appropriate use of resources and post-hospital planning balanced with the patient’s right to self-determination.
