Transitional Care Unit
Transitional care/skilled nursing helps to meet the needs of patients who no longer require acute hospital care, but still need nursing and inpatient treatment because of complications or unresolved medical conditions. Therapy is delivered at a lower intensity than acute rehabilitation.
Our Approach To Care
An individualized treatment program is developed for each patient based on an evaluation at time of admission.
The treatment team may consist of a physician, rehabilitation nurses, physical therapists, occupational therapists, speech/language pathologists, a social worker, a dietitian and an activities coordinator.
Program goals may include:
- Increased independence with the activities of daily living
- Increased mobility
- Self-management of a disease
- Home evaluation
- Patient and family education
